Why does insurance acceptance vary by state and location?

Insurance plans license payment to specific networks, license types, and geographies. The same carrier may be in-network in one state and out-of-network in another, or paneled with some therapists at an office but not others. We list the carriers paneled in each state, and our billing team can confirm coverage for the specific therapist you plan to see.

What are your self-pay rates?

Intake sessions are $180–$200 and regular sessions are $135–$160 (the exact rate depends on your therapist's license type). Neurofeedback sessions are $160. Payment is due at the time of service. Self-pay is often a good fit for clients without insurance, without mental health benefits, or on high-deductible plans.

Do you accept Medicare or Medicaid?

Medicare is not accepted at this time. For Medicaid, we accept the U of U Medicaid Plan and Select Health Medicaid (excluding the Select Health Signature plan). For other Medicaid networks please contact our billing team to confirm before scheduling.

What about out-of-network insurance like DMBA or TriWest?

Deseret Mutual Benefit Administrators (DMBA) and TriWest (for veterans) are currently out-of-network with Meadowbrook. We still process billing on your behalf for these plans. You may be responsible for any amount not covered by your plan.

What if I have an EAP, exchange, or clergy benefit?

Employee Assistance Programs (EAP) and Marketplace / exchange plans are reviewed on a case-by-case basis — contact our billing team with your member ID for confirmation. For Bishops and clergy members, we offer special pay arrangements; reach out and we'll discuss the details.
