    We offer special pay arrangements for Bishops and clergy members. Each situation is unique — reach out and our billing team will discuss the right fit for you.

  • EAP (Employee Assistance Programs)

    We work with many EAPs on a case-by-case basis. Contact billing with your EAP authorization code or member ID and we will confirm coverage before your first session.

  • Medicare

    Medicare is not currently accepted at Meadowbrook Counseling.

  • Medicaid

    We accept the U of U Medicaid Plan and Select Health Medicaid. The Select Health Signature plan is not accepted. For other Medicaid networks please contact our billing team to confirm.

  • DMBA & TriWest (out-of-network)

    Deseret Mutual (DMBA) and TriWest (for veterans) are currently out-of-network. We still process billing on your behalf — you may be responsible for any portion not covered by your plan.

  • Marketplace / Exchange plans

    Exchange and Marketplace plans (e.g. ACA plans bought directly from a carrier) vary widely. Email us your member ID and we will help verify mental health benefits before scheduling.

  • Meadowbrook Counseling is paneled with many major carriers, but acceptance varies by state, office, therapist, plan type, and service.   • Call the member-services number on the back of your insurance card and ask: (1) is outpatient mental health covered, (2) what is my copay or coinsurance, (3) do I have a deductible to meet first, (4) do I need a referral or prior authorization, (5) is telehealth covered. You can also email office@meadowbrookcounseling.com and our team will help verify your benefits.

  • Insurance plans license payment to specific networks, license types, and geographies. The same carrier may be in-network in one state and out-of-network in another, or paneled with some therapists at an office but not others. We list the carriers paneled in each state, and our billing team can confirm coverage for the specific therapist you plan to see.

  • Intake sessions are $180–$200 and regular sessions are $135–$160 (the exact rate depends on your therapist's license type). Neurofeedback sessions are $160. Payment is due at the time of service. Self-pay is often a good fit for clients without insurance, without mental health benefits, or on high-deductible plans.

  • Medicare is not accepted at this time. For Medicaid, we accept the U of U Medicaid Plan and Select Health Medicaid (excluding the Select Health Signature plan). For other Medicaid networks please contact our billing team to confirm before scheduling.

  • Deseret Mutual Benefit Administrators (DMBA) and TriWest (for veterans) are currently out-of-network with Meadowbrook. We still process billing on your behalf for these plans. You may be responsible for any amount not covered by your plan.
